Does your insurance plan require you to have a referral before acupuncture, chiropractic or massage benefits are covered?

If so, follow these instructions:
 

	Print a copy of our referral pad.
	Take the referral to your doctor and have them fill it out and sign it.
	Call our office at (503) 492-2625 to schedule your appointment and mention that you have a referral from your doctor.
	Come to your appointment 10-15 minutes early to fill out paperwork.
