  • New patients: we know it seems like a lot of work, but the information we gather before your initial visit sets the tone, pace, and healing direction your treatments will take. The more complete your paperwork, the more tailored your treatment plan becomes.

  • Existing patients: if you are already a member of the Balance family and have something new for us to help you with, you’ll want to fill out these forms at home (or work!) before coming in so that we can get a clear idea of your current health picture.

  • Auto accident? If you’ve been involved in an auto related accident your services are completely covered at Balance. Whether you were the driver, a passenger, or a pedestrian, there is no need to suffer anymore. We take care of everything from your health to dealing with the insurance companies and attorneys.

Charitable Donations Policy

Balance is very active in the community, giving back through programs and outreach. Since 2013 we have helped to raise over $15,000 for My Father’s House and Snowcap, two local charities which service east county. If you would like Balance to consider supporting your organization, please see our Charitable Donation Policy to see if we are a match. All requests for donations, regardless of time, money, or effort, must be received by the Clinic Director no less than 5 weeks prior to the desired delivery date.
